May be there is a murmur. So let us think. Rash + Murmur. Carcinoid ? Infective Endocarditis ?

Right sided murmur will go with carcinoid syndrome with pellagrous dermatitis.Skin rash is typical of pellagra. Mexico is important clue because pellagra is rare there even though maize is staple diet.
The reason is because of different way in which the grain is processed in Mexico.
